Important Health Coverage Tax Documents

You may request a copy of your Form 1095. Form 1095 provides information about the health insurance coverage offered to employees and their dependents. It also includes coverage information for individuals who enrolled in the health plan. To request a copy be mailed to you, please contact us by email, phone or mail. We will furnish your Form 1095 by March 3rd or thirty days after the date of your request, whichever is later.
